About the Role

- $59.97- $60.49 (DOE) plus 12% superannuation.
As a Clinical Care Coordinator, you will provide clinical leadership and coordination to support the delivery of high-quality, person-centred care for our residents. You will work closely with residents, families, Registered Nurses, care staff, GPs, allied health professionals and external providers to ensure care plans are current, clinical documentation is accurate, care outcomes are monitored, and services are delivered safely and effectively.

Key Responsibilities
- Lead, review and maintain person-centred clinical care plans in consultation with residents, families and the clinical team.
- Coordinate clinical assessments, referrals, appointments and follow-up actions to support safe and timely care delivery.
- Monitor changes in residents’ health, wellbeing and care needs, escalating clinical concerns where required.
- Liaise with GPs, allied health professionals, families and external providers to support integrated, evidence-informed care.
- Maintain accurate clinical documentation, progress notes and records in line with organisational and regulatory requirements.
- Support compliance with the Aged Care Quality Standards, AN-ACC requirements, privacy obligations and workplace health and safety procedures.
- Provide coaching, mentoring and practical clinical guidance to Registered Nurses, Enrolled Nurses and care team members.
- Respond to resident and family enquiries with empathy, professionalism and a solutions-focused approach.
About You
- Current Australian registration as a Registered Nurse with AHPRA.
- Minimum of 2 years’ experience as a Registered Nurse, ideally within residential aged care.
- Strong clinical assessment, care planning, documentation and AN-ACC experience.
- Experience supporting clinical education, coaching or mentoring will be highly regarded.
- Strong understanding of person-centred care, dignity of risk and the needs of older people.
- Excellent communication, organisation, clinical judgement and problem-solving skills.
- Ability to build trusted relationships with residents, families, multidisciplinary teams and external providers.
- Ability to complete pre-employment medical, National Police Check and NDIS Worker Screening Check.
