We are seeking a Care Coordinator (EEN or AIN), to join our team at the Sacred Heart Monastery Assisted Living facility on a part time basis. This is a vital position within our senior care operations that sits at the heart of the care delivered. As the Care Coordinator, with the other nursing staff and carer, you will play a pivotal role in ensuring seamless coordination of care services, supporting both the Priests and Brothers.

Key responsibilities

- Coordinate and drive the men to appointments, ensuring best care is arranged, medications delivered as ordered and the care is documented.




- Communicate effectively with care staff, nurses, allied health professionals and external service providers to ensure quality care delivery.
- Assess residents' changing needs and coordinate appropriate responses, liaising with the Director of Care and RN, to adjust care provisions as required.
- Participate in handover meetings to ensure continuity of care.
- Identify and escalate any issues or safeguarding concerns to appropriate management
- Contribute to a positive, enjoyable and inclusive workplace culture that reflects our values of compassion and respect.

Pay: $35.00 – $45.00 per hour
