Country to Coast Home Care Services is seeking an experienced Registered Nurse to conduct clinical assessments for older people receiving services under the Support at Home program. This is a adaptable, casual position working one to two days per week. The role focuses on clinical assessment and review rather than ongoing care planning or regular nursing service delivery.
Key responsibilities
- Conduct comprehensive clinical assessments in participants' homes
- Identify clinical risks, health concerns and changing care needs
- Complete clear and accurate clinical documentation
- Make appropriate recommendations and referrals
- Communicate assessment findings to the Clinical Care Coordinator and relevant team members
- Support safe, person-centred and evidence-based service delivery
- Escalate urgent clinical concerns appropriately
About you
- Current registration as a Registered Nurse with AHPRA
- Relevant clinical nursing experience
- Strong assessment, communication and documentation skills
- Understanding of clinical risk and safeguarding older people
- Current National Police Check
- Current driver's licence and reliable vehicle
- Ability to work independently in a community-based environment
- Experience in district nursing, community nursing, aged care or a similar assessment-based role would be highly regarded
