Clinical Team Leader – HASI Plus; Carlingford

About the role

- Full-time; Max-term for 6 months with possibility of extension.
- Based at Carlingford, 76 hours per fortnight with community outreach
- HPA Level 4 $61.56per hour + Salary packaging benefits + Super
- Manage on-call approximately once every month for one week, with an additional on-call allowance

This is an exciting opportunity for an experienced mental health professional to provide clinical leadership within the HASI Plus program, supporting people living with complex mental health needs to achieve greater stability, independence and connection within their community.

As Clinical Team Leader, you will provide clinical and practice leadership to a multidisciplinary team delivering recovery-oriented, person-centred support in partnership with consumers, families, carers and other services.

You will play a key role in strengthening clinical practice, supporting the team to manage complex presentations, and ensuring people receive safe, responsive and high-quality support that promotes recovery, choice and community participation.

What you will do

- Provide clinical oversight and leadership for a caseload of approximately 10–12 consumers with severe and complex mental health needs.
- Attend daily clinical handovers and lead 13-week reviews, care coordination meetings and case reviews.
- Support clinical decision-making through risk assessment, care planning and escalation.
- Provide clinical and operational supervision, training and mentoring to team members.
- Work closely with Team Leaders, multidisciplinary teams and LHD case managers to coordinate effective care and support.
- Build strong internal and external partnerships to support consumers through their transition from the facility into community-based living.
- Contribute to clinical governance, quality improvement and safe,



recovery-focused service delivery.
- Participate in the on-call roster approximately once a month for one week, with an additional allowance

About the program

HASI Plus is a residential rehabilitation mental health program provides intensive community-based support to people living with severe and persistent mental health challenges.

The program works alongside consumers to support recovery, wellbeing and greater independence, while strengthening connections with health, housing, social and community services.

The HASI Plus model recognises that recovery looks different for everyone. The team works collaboratively with each person to understand their goals, build on their strengths and support them to live the life they choose within their community.

About you

- Relevant tertiary qualification in Mental Health, Social Work, Psychology, Occupational Therapy, Nursing or a related discipline, with appropriate professional registration/membership.
- Extensive mental health experience, including experience in an inpatient setting.
- Experience supporting people with severe and persistent mental health needs, including complex presentations and risk.
- Experience with CTOs and/or Forensic Orders and some exposure to AOD.
- Demonstrated clinical leadership, supervision, care planning, risk assessment and case coordination skills.




- Strong communication and stakeholder engagement skills, including working with LHD case managers and multidisciplinary teams.
- Commitment to recovery-oriented, trauma-informed and person-centred practice.
- Current Australian driver’s licence and willingness to participate in monthly on-call roster, with additional allowance.
