Experienced General Practice Nurse – 100% Work From Home

Temporary 3-Month Position | Potential for Extension | Rural & Remote General Practice

Are you an experienced General Practice Nurse looking for the flexibility of working from home while making a real difference to patients in a rural and remote community?

We are seeking a motivated and experienced General Practice Nurse to join our rural general practice team in a 100% remote, work-from-home position.

This role would suit a nurse who has strong general practice experience, particularly in Chronic Disease Management (CDM), and who is confident working autonomously while remaining connected to the wider clinical team.

About the Role

This is initially a 3-month temporary position, with the potential to extend for the right candidate if the arrangement is successful and continues to meet the needs of the clinic and our patients.

Working remotely, you will support our patients and clinical team with a strong focus on chronic disease management, preventative health and ongoing patient care.

You will be expected to work independently, manage your workload effectively and demonstrate a high level of professionalism, accountability and reliability.

Key Responsibilities

- Provide high-quality nursing care to patients within a general practice setting

- Support Chronic Disease Management, including care planning, patient reviews, education and follow-up

- Conduct patient consultations and assessments within your scope of practice

- Assist with preventative health activities and health promotion

- Support the coordination and follow-up of patient care

- Maintain accurate and timely clinical documentation

- Liaise effectively with GPs, practice staff and other healthcare providers





- Identify and escalate clinical concerns appropriately

- Work within relevant clinical guidelines, policies and professional standards

- Maintain patient confidentiality and privacy at all times

- Contribute positively to the delivery of quality patient-centred care

About You

We are looking for someone who is:

- A Registered Nurse or Enrolled Nurse with current AHPRA registration

- Experienced working in Australian general practice

- Highly experienced in Chronic Disease Management

- Confident working independently and managing a remote workload

- Self-motivated, organised and able to prioritise effectively

- Reliable, trustworthy and accountable

- An excellent communicator who can build rapport with patients remotely

- Comfortable using clinical and practice management software

- Able to work collaboratively as part of a remote clinical team

- Committed to providing high-quality, patient-centred care

- Adaptable and comfortable working within a rural and remote healthcare environment
