We provide specialised disability support services including Supported Independent Living (SIL), community participation, in-home support, Short-Term Accommodation (STA), Medium-Term Accommodation (MTA), respite and complex support services.

We are seeking a friendly, reliable and motivated Community Care Worker to join our team in Buronga, New South Wales.

About the Role

As a Community Care Worker, you will support participants to access and participate in their community, attend appointments, engage in social and recreational activities and develop greater independence outside the home.

You will build positive relationships with participants while providing practical assistance and encouraging meaningful participation based on their individual interests, goals and preferences.

Key Responsibilities

- Provide person-centred community-based support to NDIS participants.

- Support participants to access and participate in their local community.

- Accompany participants to medical, allied health and other appointments.

- Support participants to attend community groups, events, activities and social outings.

- Assist participants with shopping, errands and other community-based activities.

- Support participants to maintain social connections, friendships and community involvement.

- Provide transport and community access where appropriate and authorised.

- Support participants to build confidence and independence when accessing community facilities and services.





- Encourage participation in meaningful activities based on each participant's interests and goals.

- Support participants to achieve their NDIS goals relating to community participation and social inclusion.

- Provide practical assistance during community outings and activities.

- Assist with daily living activities where required.

- Follow individual support plans, behaviour support plans and organisational procedures.

- Complete accurate progress notes, incident reports and other required documentation.

- Report changes in participant wellbeing, incidents or concerns appropriately.

- Maintain professional boundaries and respect each participant's privacy, dignity, choices and rights.

- Communicate effectively with participants, families, support coordinators and the Value Care team.

- Promote a safe, respectful and inclusive environment at all times.

About You

- Remain calm and professional when responding to challenging situations.

- Be flexible and willing to work across a range of shifts, including weekends where required.

Essential Requirements

- Certificate III in Individual Support, Disability, Community Services or a relevant qualification.

- Minimum 1–2 years' experience in disability, community services, aged care or a related support role.

- Current NDIS Worker Screening Check or willingness to obtain.

- Current Working with Children Check or willingness to obtain.

- Current Police Check or willingness to obtain.

- Current First Aid and CPR certificates, or willingness to obtain.

- Valid Australian Driver Licence.

- Own reliable and insured vehicle, essential for community access support.

- Ability to provide satisfactory employment references.
