Job Description

The Company: Innovative Australian Care is a registered NDIS provider. As a trusted NDIS provider, we deliver high-quality, person-centered care tailored to each participant’s unique needs and goals. The Role: As our Community Worker, you will deliver community-based support services and implement programs aimed at improving the social, emotional, and practical wellbeing of individuals and families. Responsibilities:
Assess clients’ needs and identify appropriate support services in line with individual circumstances
Develop, implement, and review individual support plans in collaboration with clients and relevant stakeholders
Provide information, guidance, and referrals to community services, including health, housing, and social support services




Support clients in accessing community resources, programs, and government services
Liaise with healthcare providers, social workers, and other service agencies to coordinate care and support outcomes

Skills & Attributes:
Relevant qualification
Minimum 1 year of relevant experience
Strong interpersonal and communication skills
Ability to work independently and as part of a team
Demonstrated empathy, patience, and commitment to client wellbeing
Knowledge of NDIS framework and person-centred care principles (preferred)
