TeamHEALTH is seeking a Housing Coach to support tenants exiting homelessness, stabilise, and move into sustainable housing across multiple programs.
The role applies recovery-oriented and trauma-informed practice to help individuals identify strengths and connect with wraparound supports.
The Housing Coach will manage a caseload, develop individual action plans, and assist in access and referral to services to achieve independent living, working across Housing Programs with a rotating roster
