Role Description:
The Care Manager is a full-time, on-site role based in Hervey Bay, QLD, responsible for coordinating and providing clinical leadership for older Australians living in our care community. Day-to-day tasks include clinical governance, conducting assessments of clinical operations, developing and updating individualized care plans, and monitoring client progress to ensure needs are met safely and effectively. The role involves liaising with families, healthcare professionals, social service providers, and internal teams to align care strategies and resources. The Care Manager will also guide and support RNQCC (similar to CCC), Nursing staff, direct care staff, ensure compliance with relevant standards and policies,



manage documentation and reporting, and contribute to continuous improvement initiatives within the service.
Qualifications
Robust clinical case management skills, including assessment, care planning, trend analysis, leadership qualities and coordination of Aged care related services.Experience in elder care and familiarity with the needs, rights, and preferences of older adults.Background in Aged care senior clinical role, with the ability to connect clients to appropriate community supports.Knowledge of medication management, National quality indicators, Aged care quality standards and ither relevant to aged care, with the ability to interpret clinical information and collaborate with medical professionals.Ability to provide training and guidance to clinical staff to support best practices and quality standards.Current AHPRA registeration without any condition as RN Div-1.Excellent communication, documentation, and interpersonal skills, with a collaborative and respectful approach.Strong organizational skills, attention to detail, and the ability to manage multiple cases and priorities.Understanding of aged care regulations, quality frameworks, and safeguarding principles.Excellent computer literacy and understanding of digital technology used in health care industry.
