A well-established, medium-sized residential aged care home is looking for a
clinically strong Clinical Care Coordinator
to join a supportive and experienced team.
This is a home where you won't be expected to do everything on your own. You'll work closely with the Facility Manager and wider clinical team, with plenty of support available from Head Office across clinical governance, quality and operations.
They are looking for someone who genuinely enjoys being on the floor, supporting their nurses and making sure residents are receiving consistently high-quality care.

About the RoleAs Clinical Care Coordinator, you'll take a hands-on role in overseeing the clinical care of residents while supporting and developing the nursing team.
You'll work closely with the Facility Manager, Registered Nurses, care staff and Head Office to maintain strong clinical standards across the home.
Education will also be an key part of the role. You'll identify areas where the team needs additional support and provide practical education and coaching to help build their clinical capability.
Key Responsibilities
Lead and oversee clinical care across the home
Support RNs and care staff with complex clinical matters
Review care plans, assessments and clinical documentation
Provide hands-on education, coaching and guidance to the team
Monitor incidents, clinical risks and changes in resident care needs
Support continuous improvement and accreditation readiness
Work closely with families, GPs and allied health professionals
Partner with Head Office clinical and quality teams to maintain standards
About You
You are a confident and capable clinician who enjoys being visible on the floor and supporting others to deliver excellent care.
You will also bring:
Current AHPRA registration as a Registered Nurse
Strong clinical experience within residential aged care
Previous experience as a CCC, Clinical Lead, Senior RN or similar
Confidence managing complex clinical care and clinical deterioration
Experience providing education or coaching to nursing and care teams
Good knowledge of aged care standards and clinical documentation
