• Lead expert assessment, care coordination and clinical advice • Build rapid assessment and hospital avoidance pathways • Support teams through clinical leadership and education • Strengthen partnerships across acute and community services About You To thrive in this role, you'll bring strong clinical expertise, leadership capability and a commitment to integrated care.
