About Us
Upwards Living is a committed provider of health and support services under the National Disability Insurance Scheme (NDIS) and Aged Care. We offer person-centred support to individuals of all ages - focusing not only on physical needs, but also on uplifting the overall quality of life for people with disability and their loved ones.
What You'll Do
Support participants with daily living activities, such as personal care, dressing, grooming, and meal preparation.
Assist participants to develop and maintain independent living skills in line with their NDIS goals.
Provide support with medication in accordance with medication policies, care plans and relevant training.
Support participants with community access, appointments, shopping, social activities and transport where required.
Follow each participant's support plan, behaviour support plan, risk assessments and individual routines.
Maintain a safe, clean and comfortable home environment, including assisting with cooking, cleaning and laundry.
Complete accurate case notes, progress notes, incident reports and other required documentation.
Observe and report any changes in a participant's wellbeing, behaviour, health or support needs to the appropriate manager.
Respond appropriately to incidents, emergencies and challenging behaviours, following organisational procedures.
Promote participants' choice, dignity, privacy, rights and independence at all times.
Work collaboratively with other Support Workers, Senior Support Workers, Service Coordinators, families, allied health professionals and other stakeholders.
Participate in shift handovers and communicate important information to ensure continuity of support.
Follow NDIS Practice Standards, workplace health and safety requirements, infection control procedures and organisational policies.
Be able to work
sleepover shifts
at least
32 hours per week

Have previous experience working in Supported Independent Living environment and supporting clients who require intensive care
Strong communication skills in English (speaking and writing) and Chinese or Vietnamese
Certificate IV or above in Disability, Aged Care, or Community Services. If you have a Certificate III, you're required to have at least 2 years of experience working in SIL environment
NDIS Worker Screening
Working with Children Check
Current First Aid and CPR certificate
Valid Working Rights to work in Australia
Current valid driver's licence and access to a reliable vehicle
Availability and Location
Monday 6:00pm - 7:00am in Liverpool
Thursday 6:00pm - 7:00am in Liverpool
Saturday 6:00pm - 7:00am in Liverpool or 3:00pm - 7:00am in Bass Hill
Sunday 6:00pm - 7:00am in Liverpool or 10:00pm - 6:00am in Bass Hill or 10:00pm - 10:00am in Chester Hill
This role strictly requires working sleepovers shifts at least 32 hours per week, so please ONLY apply if you meet the required working hours as outlined above.
