Role Responsibilities
Coordinate formal and informal supports
Manage clients with complex needs and handle crisis situations
Collaborate with a Multidisciplinary Panel
Engage with Mental Health, Justice System, Housing, and Health sectors
Undertake clerical administration and report writing
Network and communicate with key stakeholders
Work independently
Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.
Qualifications & Experience
Minimum of Certificate IV in Community Services or related discipline
Minimum 2 years' experience working with participants with moderate to complex mental health issues, high medical needs, and/or behavioural support needs
Experience in Forensic Disability or Forensic Mental Health is a plus
Demonstrated in-depth knowledge of the NDIS and experience working with its participants
Valid Driver's License with a suitable, insured vehicle
A current Working with Vulnerable People registration
Recent police check (or willingness to obtain one)
Competitive remuneration ($78,000 – $89,000 per year, plus Superannuation)
