Job Description
As a Supportive Care Navigator, you'll be part of the Leukaemia Foundation's Triage & Navigation team, handling inbound referrals and phone calls from individuals and healthcare professionals. By conducting comprehensive intake assessments, you'll offer initial support and connect clients to relevant information and ongoing supportive care.
Your mission is to appropriately triage, coordinate, and facilitate client access to the Leukaemia Foundation's full suite of supportive care services, including digital, 1:1, and practical supports such as financial assistance.
You'll work collaboratively with healthcare professionals, both internal and external, and maintain accurate, up-to-date healthcare records that comply with organisational policies, regulatory requirements, and health service standards.

This role is funded through the Australian Cancer Nursing and Navigation Program (ACNNP)
This role can be based out of our Perth, Adelaide, or Artarmon office, with Herston Village (Brisbane)



being the preferred location in Queensland
You'll need to be available between 9.00am and 4.00pm AEST, Monday to Friday (including if you're based in another timezone)
What you'll be doing
Receive, manage, and document all inbound referrals and phone calls from individuals and healthcare professionals relating to supportive care services.
Conduct thorough risk assessments and supportive care needs assessments, prioritising client needs to determine the most relevant services.
Provide clients with accurate information about blood cancer and supportive care services, including relevant external supports.
Triage, coordinate, and facilitate client access to supportive care services, including digital, 1:1, and practical supports such as financial assistance.
Work collaboratively with healthcare professionals, internal and external, and maintain accurate, up-to-date healthcare records.
What we are looking for:
Tertiary qualification in social work, nursing, or an allied health profession
AHPRA registration or membership of the relevant professional body
Comprehensive understanding of blood cancer and its impact on clients
Knowledge of supportive care services and resources available to blood cancer patients




Demonstrated ability to work effectively both independently and as part of a multidisciplinary team
Robust interpersonal and communication skills, with the ability to provide empathetic, compassionate support over the telephone and digital modalities
Excellent organisational and time management skills, with the ability to prioritise and manage multiple tasks
Proficiency using IT systems and maintaining detailed, accurate patient records
High level of emotional resilience and the ability to handle sensitive and distressing situations
It's a bonus if you have:
Experience in haematology/oncology settings and/or supportive care in cancer services
Training in counselling or psychosocial support
