As a Clinical Pharmacist,



you will play a critical role in improving medication safety, optimising therapy outcomes and supporting continuity of care for patients throughout their healthcare journey.
What You'll Be Doing
Conduct clinical medication reviews and medication reconciliation
Provide expert medication advice to medical, nursing and healthcare teams
Support safe and effective use of high-risk and controlled medicines
Identify opportunities to improve medication safety and patient outcomes
Participate in clinical governance and quality improvement activities
Build strong partnerships with multidisciplinary healthcare stakeholders
About You
Current AHPRA registration as a Pharmacist
Strong clinical knowledge and patient-centred approach
Excellent communication and stakeholder engagement skills
Ability to work independently across multiple locations
Strong organisational and problem-solving skills
