Bayside Health’s Caulfield Community Health Service seeks an Occupational Therapist to join the Adult Health team at Caulfield Hospital. The role focuses on home assessments, modifications and coordinated care to support independence for predominantly clients over 65 via aged care pathways.
You will work within a multidisciplinary team, develop person-centred care plans, and liaise with councils, contractors and equipment providers to deliver home modifications and equipment solutions.
