Uniting Communities in South Australia is seeking a seasoned health professional to join our Aged Care Assessment Service (ACAS) as a Triage and Clinical Assessor. You will perform phone-based triage, comprehensive clinical assessments, determine eligibility for support under the Aged Care Act, and advise on appropriate pathways for each client.
This role requires robust clinical judgement, empathy, and the ability to work with diverse communities, including Aboriginal and Torres Strait Islander
