Uniting AgeWell in Melbourne, VIC, seeks a Care Advisor to manage a caseload of older clients across three weekdays (9:00 am–5:00 pm) at the Burwood East office.
This permanent part time role focuses on developing personalised care plans to promote wellbeing, independence and meaningful outcomes.
You will coordinate services with families, health specialists and providers and support restorative, wellness-focused approaches that maximise client independence and safety at home.
