Job Description
Gladstone Region Aboriginal and Islander Community Controlled Health Service Limited (GRAICCHS), trading as Nhulundu Health Service, provides a comprehensive range of primary health care, aged care and community support services across the Gladstone and Central Queensland region.

We are seeking a motivated experienced Registered Nurse who is passionate about making a difference in health outcomes for the Aboriginal and Torres Strait Islander Community, to join our multidisciplinary team and support the delivery of high-quality, culturally safe primary health care services to Aboriginal and Torres Strait Islander peoples and the wider community.

Job Description
n
About the role:
n
As a Registered Nurse,
you will provide comprehensive primary health care services while working closely with GPs and a multidisciplinary team to improve the health and wellbeing of our community.
n
More specifically, this role will focus on key activities, including but not limited to:
n
n
- Deliver comprehensive primary health care services including triage, health assessments, wound care, immunisations and routine clinical procedures.
n
- Support chronic disease management through client assessments, care planning, monitoring and follow-up.
n
- Provide health promotion, education and preventative health initiatives to individuals, families and the broader community.
n
- Coordinate referrals and collaborate with GPs, specialists, hospitals and other health providers to ensure continuity of care.
n
- Maintain safe clinical practices, including infection control, medication management and vaccine cold chain management.
n
- Ensure accurate clinical documentation and support Medicare billing requirements.
n
- Participate in quality improvement initiatives and contribute to the ongoing enhancement of service delivery.
n
- Work collaboratively within a multidisciplinary team to achieve positive health outcomes for clients.
n
- Contributing to a positive team environment by mentoring students and colleagues, participating in quality improvement activities and maintaining professional standards.

Desired Skills and Experience
n
What we are looking for:
n
You are a compassionate and motivated Registered Nurse who enjoys working in a community-focused environment and values culturally safe care, who meet the following criteria:
n
n
- Current AHPRA registration as a Registered Nursewith no conditions or undertakings
n
- A bachelor's degree in Science - Nursing (or higher)
n
- Demonstrated clinical experience in Primary Health Care with strong assessment and nursing skills
n
- Experience in clinical or primary health care nursing
n
- Strong clinical assessment and communication skills
n
- A commitment to culturally respectful practice
n
- Current CPR and First Aid certification
n
- A current Queensland C Class Driver Licence
n
- A satisfactory Criminal History Check
n
- A current Blue Card (or willingness to obtain)
n
- Experience working within a primary health care or Aboriginal Community Controlled Health Service environment.
n
- Immunisation endorsement and experience.
n
- Experience in chronic disease management and health promotion programs.
n
- Experience using electronic patient record systems and other clinical software.
n
n
Pre-employment checks, including a National Criminal History Check, LCS2 and reference checks, will be required.
