The Patient Follow-Up Coordinator manages day-to-day administration and coordination of Remote Monitoring and Supported Self-Management Pathways (SSMP) within the Cancer Support Team. The post holder ensures accurate data entry, monitors pathway compliance, and supports quality assurance checks and data integrity.

Working with Cancer Support Workers, Cancer Nursing teams, Prehabilitation services and other multidisciplinary colleagues, the role supports protected, effective and patient-centred
