Occupational Therapist - Community Aged Care

Are you an experienced Occupational Therapist who is passionate about helping older Australians maintain their independence, safety and quality of life at home?

We are seeking a dedicated Occupational Therapist to join our growing community aged care team, delivering high-quality, person-centred services across Melbourne's Northern and Western metropolitan regions. This is an opportunity to make a meaningful difference through restorative care, reablement, assistive technology and home modification programs. Based on a flexible hybrid work model, you'll split your time between our Brunswick office, working from home, and visiting clients in their homes.

- $88,000 salary + 12% Super (pro-rata)

- $8,000 Motor Vehicle Allowance

- 5 weeks annual leave

- Hybrid work model: Brunswick office, work from home & community visits

- Support clients across Melbourne's Northern & Western suburbs

- Weekly training and team meetings
n




- 4-day week (must work Tuesdays, other days versatile)

About the Role

n

n
- Conduct home-based occupational therapy assessments
n
- Deliver reablement and restorative care programs
n
- Recommend assistive technology and home modifications
n
- Prepare clinical reports and funding documentation
n
- Work closely with clients, families and the broader care team to achieve positive outcomes.
n

About You

n

n
- Degree in Occupational Therapy
n
- Current AHPRA registration
n
- Minimum 3 years' community aged care experience
n
- Knowledge of Home Care Packages and Support at Home programs
n
- Current driver's licence and reliable vehicle
n
- Excellent communication and report-writing skills.
n
