Bayside Health is seeking a Social Worker (Grade 1-2) to support Acquired Brain Injury patients at Caulfield Hospital, part of the Alfred Care Group. The fixed term parental leave role is full time (80 hours per fortnight) with adaptable monthly ADO.

You will work with clinical units to identify needs, manage complex cases and liaise with NDIS, TAC, Workcover, TCP, Carer Services, OPA and VCAT, within an inter-disciplinary team to improve patient care and transitions.
