Country To Coast Home Care Services – Anglesea VIC

You will provide person-centred support guided by each participant's care plan, goals, preferences and assessed needs. Work is community-based and may involve travel between participants across Aireys Inlet, Anglesea, Torquay, Jan Juc, Bells Beach and Bellbrae.

Monday, Wednesday and Friday, 8:00 am–5:00 pm

About the role

n

You will provide person-centred support guided by each participant's care plan, goals, preferences and assessed needs. Work is community-based and may involve travel between participants across Aireys Inlet, Anglesea, Torquay, Jan Juc, Bells Beach and Bellbrae.

What you may do

n

n
- Personal care, including assistance with showering, dressing and grooming
n
- Domestic assistance and light household tasks
n
- Social support, companionship and assistance with community participation
n
- Meal preparation and shopping assistance
n
- Transport to appointments, shopping and community activities
n
- Observe and report changes in a participant's wellbeing, needs or circumstances
n




- Complete accurate service records and follow care plans, policies and safe work practices
n

What you will bring

n

n
- Certificate III or IV in Individual Support (Ageing), Aged Care or a relevant qualification, or currently working towards one
n
- A current National Police Check and any other screening required for the role
n
- A current driver licence and a reliable, comprehensively insured vehicle
n
- A current First Aid and CPR certificate, or willingness to obtain these paid for by the organization
n
- Robust communication skills, empathy, reliability and respect for participant choice, privacy and dignity
n
- Confidence working independently in participants' homes and using mobile systems for documentation
n
