Central Adelaide Local Health Network is seeking a Senior Pharmacist for the Transition Care Program in Adelaide. You will lead a pharmacy team, provide high‑level clinical services and contribute to improving outcomes for older patients transitioning from hospital to home.

You will oversee medication histories, safety documentation and audits while collaborating with medical and nursing staff to ensure cost‑effective, protected pharmacotherapy across Residential Care Facilities.
