We are seeking an experienced and motivated Support Coordinator to join our team.

n

This Position requires an Individual who has:

n

n
- Experience providing NDIS Support Coordination/Recovery Coaching Services or Similar Role
n
- Support participants to understand and implement their NDIS plans
n
- Assist participants in building capacity, independence, and decision‐making skills
n
- Coordinate and connect participants with appropriate service providers
n




- Monitor supports and ensure services align with participant goals and funding
n
- A Solid understanding of the NDIS and its Importance to People with a Disability and their Support Networks
n
- Demonstrated experience with Facilitating Multi‐Disciplinary Coordinated Planning and Develop Cooperative Relationships and Networks between Service Providers
n
- Maintaining an Up‐To‐Date knowledge of the NDIS Systems and Practices
n
- Engaging with NDIA Staff and LAC's
n
- Liaising with the Participant, Family or Representative on Supporting the Participant with Achieving their Goals
n
- Preparing for Plan Reviews and Attending with the Participant, Family or Representative
n
- Maintain accurate and compliant documentation in line with NDIS requirements
