Care Coordinator
n
About the Role
n
As a Care Coordinator,
you'll be the key link between clients, families, and care workers. Reporting to the Branch Manager, you'll ensure care is delivered smoothly, safely, and consistently — while supporting and developing our frontline teams.
n
Your Key Responsibilities
n
Create and manage staff rotas, ensuring appropriate travel time and breaks
n
Match carers to clients based on skills, preferences, and location
n
Manage recent referrals from private clients and social services
n
Support assessments, care planning, and care package reviews
n
Communicate with clients, families, and healthcare professionals
n
Carry out risk assessments, reviews, and care worker supervisions
n
Ensure care documentation and client records are accurate and up to date
n
Support weekly KPIs, logbook drops, and team meetings
n
Organise and participate in supervisions, appraisals, and training
n
Take part in the on-call rota, including alternate weekends
n
Provide occasional support to neighbouring branches when required
n
