As a Practice Leader with Possability, you will support and guide Disability Support Workers to deliver high quality, person-centred supports that improve quality of life for the people we support.
n
This role combines hands-on support with team leadership, where you will model best practice, coach staff and build a strong, collaborative team focused on positive outcomes for customers.

What you will be doing
n
n
- Leading and mentoring Disability Support Workers to deliver quality supports
n
- Observing, modelling and coaching best practice support approaches
n
- Supporting customers to achieve their personal goals and build independence
n
- Facilitating team meetings and encouraging teamwork and collaboration
n
- Monitoring risks and identifying opportunities to improve outcomes
n
- Supporting the implementation of behaviour support and health support plans
n
- Ensuring services meet quality standards and NDIS requirements
n
- Assisting with onboarding and mentoring new team members
n
- Completing documentation including reports,
incident records and audits
n
- Working directly alongside customers and modelling high quality support delivery
n

What you will bring

- Experience supporting people with disability, particularly those with complex needs
n
- Demonstrated leadership or mentoring experience within support teams
n
- Strong communication and interpersonal skills
n
- Ability to coach, guide and motivate team members
n
- Robust organisational and problem solving skills
n
- Confidence using systems to complete documentation and reporting
n
- Current NDIS Worker Screening Clearance or willingness to obtain
n
- Current drivers licence
n
- Relevant qualifications in Disability, Community Services or similar are desirable
n
