We are currently looking to recruit a part time Band 5 Mental Health Practitioner (Mental Health Nurse, Occupational Therapist or Social Worker) to join our small and supportive Community Mental Health Team for older adults. This is a outstanding opportunity to join our friendly and dynamic multidisciplinary team, with lots of support and CPD opportunities.

We provide patient focused, evidence-based mental health interventions and assessment for people aged 65 and above, with a range of Functional and Organic mental health illness.
n
We are looking for people who have a relevant NMC or HCPC registration and:
n
n
- Relevant pre-registration experience of working in mental health environments, however this is not essential.
n
- Previous experience and skills which can transfer into this role – good communication, organisational skills and timekeeping amongst others.
n
- We welcome applications from people with lived-experience of mental health challenges.

- People who are passionate about working with older people in the community experiencing mental health challenges.

n
The post is based at Brookfields Hospital. CPFT supports agile working, with flexibility for work-life balance. Visits to the team are welcome. There is free parking onsite.
n
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.

The role involves (but is not limited to):
n
n
- Providing support interventions for people with significant mental health conditions.
n
- Working within a multi-disciplinary team with colleagues in the wider Older People's Mental Health Team.
n
- Monitoring and reviewing the use of prescribed medications.
n
n
Your primary area of coverage will be Central Cambridge , but you must also remain flexible to support other parts of Cambridge as needed. Regular travel will be essential for this role. We provide a preceptorship programme and regular support and supervision.
n

- 1.To provide an ongoing assessment of the care needs, plan care in collaboration with the service user, their carers (where appropriate) and MDT colleagues. Deliver care in a safe and effective manner and ensure that needs are reviewed on a timely basis commensurate with service user needs.
n
- 2.Supporting service user medication concordance in association with MDT colleagues.
This will be achieved by demonstrating a working knowledge of medications and policies commonly used in older people's mental health services including relevant NICE guidance e.g., anti-psychotic prescribing in dementia care
n
- 3.To actively engage in partnership working in respect of the patient, carers and service development
n
- 4.To have a working knowledge of current mental health and wider legislation, in addition ensuring effective application of CPA assessment and care planning principles and effective risk assessment.
n
- 5.Contribute to the assessment, treatment, evaluation and effectiveness of treatment / care offered and review plan with multi -professional team / team manager.
n
- 6.Liaise effectively with other members of the service team, primary health care teams, residential/nursing homes, adult social care and other organisations contingent upon the client.
n
- 7.Employ specific skills to help resolve/manage problems in older persons mental health e.g. anxiety management strategies, behavioural management strategies in dementia, family support skills etc.
n
- 8.Be familiar with and observe statutory and local policies in the prescribing, custody and administration of medications in line with the Trust's medicine management policies and procedures.
n
- 9.Maintain a high standard of written reports, records of visits/interventions, care plans recording the progress of clients in line with standards for records and record keeping
n
