Ballarat Hospice Care is seeking a compassionate and skilled Supportive Care Advisor to join our multidisciplinary team providing specialist, home-based palliative care. In this rewarding role, you will provide psychosocial, emotional, spiritual, and bereavement support to patients, families and carers receiving specialist palliative care at home.

About the role
n
You will undertake assessments, provide counselling, contribute to care planning, and work closely with clinical staff and volunteers to support holistic, person-centred care.
n
You may also contribute to community events, education sessions and workshops that promote understanding of palliative care.
Some planned out-of-hours activities may be required.

About you
n
You will bring experience in counselling (or similar), social work or pastoral care, and a commitment to holistic, person-centred practice. You are empathetic, professional, and able to work collaboratively within a multidisciplinary team.
