About the Role
Reporting to the Operations Manager, you’ll coordinate and oversee participant supports, ensuring services run smoothly, staff are supported, and outcomes align with client goals and NDIS plans.
This hands-on role involves direct contact with participants, families, and staff across multiple sites, helping resolve day-to-day issues, coordinating rosters, ensuring quality documentation, and maintaining compliance. You’ll be the go-to person for ensuring that every participant’s supports are person-centred, consistent, and delivered to a high standard.
Key Responsibilities
• Coordinate daily supports for participants across both community and SIL settings.
• Build and maintain strong, respectful relationships with clients, families, and guardians.
• Oversee participant onboarding, transitions, and changes to support needs.
• Work closely with the Rostering team to ensure adequate staffing coverage across shifts.
• Monitor and review participant progress, updating support plans and documentation as required.
• Respond to and document incidents, behaviours of concern, or restrictive practices in line with NDIS and organisational procedures.
• Support staff in implementing behaviour support plans and ensure consistency of practice across teams.
• Conduct site visits to check on participants’ wellbeing and quality of service delivery.
• Ensure daily notes, progress reports,
and SIL documentation are accurate and up to date.
• Liaise with allied health professionals, external providers, and NDIA representatives as needed.
• Provide coaching and guidance to support workers to improve service quality and communication.
• Contribute to continuous improvement, identifying and addressing gaps in service delivery.
• Promote a positive, safe workplace culture and uphold WHS and risk management procedures.
What You’ll Bring
• Certificate III or IV in Individual Support, Disability, Community Services or equivalent.
• NDIS Worker Screening Check.
• Working with Children Check
• Previous experience coordinating or delivering disability services, preferably within SIL or community settings.
• Sound understanding of the NDIS framework, participant plans, and compliance reporting.
• Strong organisational skills and confidence working in a rapid-paced, people-focused environment.
• Excellent written and verbal communication skills, with a compassionate and proactive approach.
