Monash Health is seeking a Credentialled Diabetes Nurse Educator for a part time permanent role across community sites including Cranbourne Community Hospital. You will deliver diabetes education, assess and treat patients, and coordinate care with a multidisciplinary team.
Applicants should have the Post Graduate Certificate of Diabetes Education and be an ADEA Credentialed Diabetes Educator, with a minimum of 3 years’ experience in diabetes education.
