About the role
The purpose of this role is to provide support coordination to National Disability Insurance Scheme (NDIS) clients and assist them in achieving their goals outlined in their funded plan.
A Support Coordinator's role is to proactively identify, develop, coordinate, and facilitate access to supports and services alongside their allocated NDIS clients.
The position requires the ability to deliver high quality, culturally appropriate and effective services across a variety of settings.
Key responsibilities
Using a person-centred approach, support and encourage clients to build capacity to achieve their goals and provide support coordination services in line with individual NDIS plans.
Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports, including preparing for and chairing stakeholder meetings.
Build client capacity to utilise their NDIS plan by increasing their knowledge and understanding of the NDIS.
Prepare and maintain up to date and accurate case management notes and reports using clear and concise language on client progress according to required timelines.
Liaise effectively and diplomatically with a wide range of stakeholders, community services, government agencies, customers, and service providers.
About you
Demonstrated well-developed oral, written, and interpersonal communication skills.
Demonstrated ability to work well independently and cooperatively as a member of a team.
Established ability to listen actively and demonstrate empathy when confronted with adversity.
Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.
Demonstrated ability to interact and engage effectively with diverse cultures.
A willingness and ability to travel by car, 4WD, and/or light aircraft, to regularly work in rural and remote settings.
Criminal History Check, Working with Children Check, NDIS Worker Screening, Drivers Licence, Right to Work in Australia, and a suitable roadworthy vehicle insured to a minimum level covering third party fire and theft.
