Are you an experienced Support Coordinator with an established participant and referral network?
n
Ableconnect is a growing NDIS provider based in Fitzroy, Melbourne, delivering person-centred supports that empower people with disability to achieve their goals and live fulfilling lives.
n
We are seeking an experienced Level 2 and/or Level 3 Support Coordinator to join our growing team on a full-time subcontractor basis.
n
This opportunity is particularly suited to a Support Coordinator who has existing professional networks, referral relationships and the ability to build and maintain their own participant caseload.
n

About the Role
n
Based at our Fitzroy Head Office, you will manage your own participant caseload and deliver high-quality Level 2 and/or Level 3 Support Coordination in accordance with NDIS Practice Standards.
n
You will have the opportunity to:
n
n
Build and manage your own participant caseload.
n
Deliver high-quality Level 2 and/or Level 3 Support Coordination.
n
Develop strong, professional relationships with participants, families, guardians and stakeholders.
n
Support participants to understand and implement their NDIS plans.
n
Coordinate services and connect participants with appropriate providers.
n
Develop person-centred strategies to support participant goals.
n
Build and maintain referral and professional networks.
n
Identify opportunities to introduce suitable participants to Ableconnect's Support Coordination, Supported Independent Living (SIL) and Community Participation (CP) services.
n
Work closely with our Service Delivery team while maintaining appropriate professional boundaries and managing conflicts of interest.
n
Maintain accurate case notes, reports, documentation and billing.
n

We Are Looking for Someone Who:
n
n
Has a minimum of 2 years' experience as a Support Coordinator.
n
Has a strong understanding of the NDIS and person-centred practice.
n
Has an established network across the disability, community, health or allied health sectors.
n
Has experience developing referral relationships and building participant caseloads.
n
Is confident engaging with participants, families, guardians and professionals.
n
Is proactive, motivated and commercially minded while maintaining a strong commitment to participant outcomes.
n
Can work autonomously and manage a caseload effectively.
n
Has excellent communication, organisation and documentation skills.
n
Is committed to high-quality service delivery and the NDIS Code of Conduct.
n
n
This role is particularly suited to an experienced Support Coordinator who can bring established professional relationships and referral opportunities.
n
We are looking for someone who understands the local NDIS landscape and has the ability to develop a sustainable caseload through existing networks, professional relationships and new referral opportunities.
n
There is also an opportunity to contribute to the growth of Ableconnect's broader services, including SIL and Community Participation, where appropriate and in accordance with NDIS requirements and conflict-of-interest obligations.

You must have:
n
n
Australian Business Number (ABN).
n
GST registration, where applicable.
n
NDIS Worker Screening Check.
n
National Police Check with 12 months.
n
Current Victorian Driver Licence.
n
Reliable vehicle and comprehensive motor vehicle insurance.
n
Current First Aid and CPR Certificate.
n
NDIS Worker Orientation Module Certificate.
n
Supporting Safe and Enjoyable Meals Certificate.
n
