As our Care Coordinator, you will:

n
Understand the Commonwealth Home Care Support and HACC programs
n
Conduct assessments: including identifying any health risks, medical conditions, or social and environmental factors
n
Develop care plans
n
Coordinate care
n
Provide support and education.
n
n
Skills and Experiences
n
What we're looking for:
n
n
Current Australian driver's licence
n
National police check
n
Working with vulnerable people/children check
n
Minimum Certificate III in a relevant discipline
n
n
