Care Partners play an important role in supporting the safety, health and overall wellbeing of older people. Through person-centred, high-quality care management, they help ensure services reflect each participant's preferences, cultural background and support needs, enabling them to live independently in the community.
n
We currently have a full-time opportunity for an experienced Care Partner to manage a designated caseload of Support at Home (SAH) participants within our Home Care Packages Team.
n
In this role, you will onboard new participants, develop and review individual care plans, and coordinate services to meet a range of complex care needs.
n
This position combines community-based client visits with time spent in our dynamic office workplace in Pakenham.

Qualifications in Social Work, Allied Health, Community Services, or a related qualifications will be considered based on the Support at Home Manual scope of acceptable qualifications. (including Registered or Enrolled Nurses with current AHPRA registration – Division 1 or 2)
n
You'll be open to travel, balancing community visits with clients and time in a vibrant office workplace
n
Understanding of Home Care Packages/Support at Home, My Aged Care, and Aged Care Quality Standards
n
Strong commitment to supporting older adults to remain living independently in their homes
n
Well-developed communication, planning, organisational,
and time management skills
n
Proficiency in Microsoft Office and participant management systems
n
Demonstrated ability to build and maintain effective working relationships with both internal teams and external service providers
n
Current and valid First Aid and CPR Certificate
n
A current Victorian Driver's Licence
n
Experience in managing Home Care Packages/Support at Home or working within government-funded health or social service programs
n
Familiarity with Carelink Plus or similar client management software
n
Demonstrated experience working with diverse aged care populations, including those with specialised or complex care needs
n
Background in assessment, care planning, and service coordination for community-dwelling older adults
n
Proven ability to negotiate and liaise effectively with external community service providers
n
