ABOUT THE ROLE

As an Aboriginal Peer Navigator you will connect with Aboriginal and Torres Strait Islander people living with complex mental health and psychosocial challenges in the Newcastle and Hunter region. Make a meaningful difference by walking alongside community to navigate health and community systems, build on their strengths, connect with cultural supports and work towards social and emotional wellbeing goals.

DUTIES
n
n
Provide culturally safe peer navigation and psychosocial support to Aboriginal and Torres Strait Islander participants.
n
Connect participants and families with GPs, clinical, cultural community-based supports
n
Build trusted relationships with participants, families and carers to support informed choices and wellbeing planning
n
Provide practical support, outreach and support coordination for participants and their support networks with the Yudhilidin team
n
Maintain accurate client records and contribute to continuous improvement of the program
n

SKILLS & EXPERIENCE
n
n
Demonstrated experience providing culturally safe support to Aboriginal and Torres Strait Islander people
n
Relevant mental health experience, lived experience and/or qualifications (min. Certificate IV or working towards)
n
Ability to connect, engage and support people within a social and emotional wellbeing framework
n
Experience collaborating with external service providers, particularly primary care and other community-based health services
n
Solid task management skills; a positive, proactive and solutions-focused attitude
n
