About the Role
n
n




Provide outreach nursing care with a team in community settings for people experiencing homelessness or poverty.
n
Offer preventative care, health education, wound management, medication support, Hep C testing, and social referrals to health and community services.
n
Case management – Coordinate care and local referrals, link clients with appropriate services, and advocate for their health needs.
n
Work closely with social workers, community organisations, hospitals and other health professionals.
n
Participate in education, advocacy, and health promotion to improve access to care.
n

About You
n
n
Enrolled or Registered Nurse (AHPRA) with experience in primary health, outreach, or community care.
n
Passionate about social justice and supporting people experiencing disadvantage.
n
Able to work autonomously and problem‐solve in energetic environments.
n
Current driver's licence and access to a reliable vehicle (travel is required for local outreach work).
n
