The prospect
n
Reporting to the Patient Services Business Manager, you'll work within a close-knit multidisciplinary team, coordinating and supporting patients from diagnosis through treatment and beyond. Your day-to-day will include:
n
n
Providing proactive, personalised support to patients and carers navigating their cancer care pathway
n
Coordinating appointments, referrals, and communication across clinical teams to reduce gaps and delays
n
Identifying and connecting patients with psychosocial, financial, and community support services
n
Maintaining accurate case records and contributing to team reviews to improve care coordination outcomes
n
n
What You'll Bring
n
n
A background in nursing, allied health, or a related clinical discipline with current AHPRA registration or relevant qualifications
n
Experience in oncology, chronic disease management, or complex care coordination settings
n
Demonstrated ability to communicate sensitively with patients facing serious illness and their support networks
n
Strong organisational skills with the ability to manage competing priorities across a multidisciplinary environment
n
Familiarity with electronic medical records and care coordination tools
n
An understanding of the psychosocial dimensions of cancer care — something central to the Lifehouse model
n
Registration with the Australian Health Practitioner Regulation Agency (AHPRA) - if applicable
n
Legal right to work in Australia - we regret no sponsorship is offered for this position
n
Current Working with Children Check (WWCC)
n
National Police Check (within the last 6 months or willingness to obtain)
n
Compliance with Category A immunisation requirements
n
