Uniting Communities seeks an experienced health skilled to join the Aged Care Assessment Service (ACAS) as a Triage and Clinical Assessor. You will conduct phone-based triage and clinical assessments to determine eligibility for support under the Aged Care Act and identify the best assessment pathway for each client.

You will apply your clinical judgement to determine funding approvals, undertake Comprehensive and Home Support Assessments, and support plan reviews, collaborating with ACAS
