At Right at Home Ipswich, Lockyer Valley, we're looking for a Full-Time Care Co-Ordinator to join our friendly and dedicated team, supporting clients across aged care and disability services in the community.

About the Role
n
As a Care Partner, you'll be the vital link between our clients, their families, and our care team. You'll ensure that every client receives personalised, high-quality support tailored to their unique needs, helping them live safely and independently at home.
n
Key Responsibilities
n
n
Coordinate and schedule Support at Home services
n
Match clients with suitable care professionals based on skills and compatibility
n
Maintain regular communication with clients, families, and staff
n
Monitor and update client care plans and quarterly budgets
n
Ensure compliance with NDIS and Aged Care Quality Standards
n
Deliver exceptional customer service and maintain accurate records
n
n
About You




n
n
Minimum 1 year of experience in a similar Care Partner or Co-Ordination Role
n
Sound understanding of NDIS and Aged Care Quality Standards (preferred)
n
Strong organisational, time management, and communication skills
n
Empathetic, client-focused approach to care
n
Confident using scheduling software and IT systems
n
n
Qualifications
n
n
Certificate III or IV in Individual Support (Ageing or Disability), or
n
Diploma in Community Services
n
n
