About the Role
n
We're seeking an experienced Registered Nurse to join our Ballarat Medical Centre in a permanent full time dual role:
n
n
2 days per week: Treatment Room Registered Nurse
n
3 days per week: Chronic Disease Management Nurse
n
n
You'll work closely with GPs and the broader practice team to provide high-quality treatment room services while supporting patients to prevent and manage chronic conditions.
n
Key Responsibilities
n
Treatment Room Nursing
n
n
Provide vaccinations, wound care and other treatment room services.
n
Conduct health assessments and assist with minor procedures.
n




Maintain accurate clinical records and follow infection-control requirements.
n
Support GPs with day-to-day clinical care.
n
n
Chronic Disease Management
n
n
Prepare and coordinate GP Management Plans and Team Care Arrangements.
n
Educate and empower patients to manage their health conditions.
n
Coordinate referrals with multidisciplinary healthcare providers.
n
Promote preventative healthcare and sustainable lifestyle changes.
n
Review and update patient care plans.
n
Complete all associated clinical and administrative tasks accurately.
n

About You
n
n
Current AHPRA registration as a Registered Nurse.
n
Previous general practice nursing experience.
n
Experience in Chronic Disease Management and treatment room nursing.
n
Confidence completing health assessments and care plans.
n
Strong organisational, communication and time-management skills.
n
A patient-focused approach and passion for preventative healthcare.
