As aCase Manager, in our Housing Stability Team, you'll play a vital role in helping individuals and families to regain stability and rebuild their lives as part of our Supportive and Transitional Housing Program.
You'll provide personalised, wrap-around support to empower people to settle into their home after experiencing homelessness with dignity and confidence.
What You'll Be Doing
Engaging with clients in their homes, to build trust and develop tailored support plans.
Navigating complex challenges by working alongside health, legal, and social services to provide holistic, person-centred care.
Responding to crises with skill and compassion, ensuring people receive immediate support when they need it most.
Advocating fiercely for your clients, ensuring their rights, needs, and voices are heard.

Keeping clear and accurate records to support meaningful service improvements.

Is This You?
We're looking for someone who iscompassionate, adaptable, and driven to make a difference.
If you have:
Atertiary qualificationin Social Work, Social Sciences, Counselling, Psychology, or Community Services (Cert IV minimum with relevant experience).
Experience incase management, crisis intervention, and supporting individuals with complex needs.
A strong understanding ofcollaborative care, trauma-informed care, and strength-based approaches.
Excellent communication and advocacy skills.
A validSA Driver's Licence,a current National Police clearance, a Working with Children Check, anNDIS Worker Screening Check, andFirst Aid/CPR Certification(or willingness to obtain).
