The Role
n
We are looking for an experienced Part-Time Chronic Disease Management (CDM) Nurse to join our Forest Hill practice and be a driving force and advocate for preventative health within the local community.
n
This role will see you support GPs and their patients to identify and manage chronic conditions, helping patients improve their health outcomes and ultimately stay out of hospital.

The Tasks
n

Prepare Care Plans and Team Care Arrangements to help patients manage their conditions
n
Empower patients to take greater ownership of their health
n
Coach patients on embedding positive health behaviour changes
n
Enable patients to access ongoing care from a multidisciplinary team
n




Assist with the early identification of chronic disease by conducting health assessments
n
Work with GPs to promote preventative healthcare throughout the practice
n
Increase the number of patients receiving a care plan for the first time
n
Ensure patients' care plans are reviewed regularly
n
Manage administrative tasks relating to CDM appointments
n

The Successful Candidate
n
n
AHPRA-registered Enrolled Nurse or Registered Nurse
n
Proven experience in Chronic Disease Management and health assessments
n
Robust knowledge of health assessments is highly regarded
n
Excellent organisational and time management skills
n
Outstanding communication and customer service skills
n
A passion for helping people and a personable approach
