You will be part of a service that truly bridges hospital and community care, supporting patients to receive the right care, in the right place, at the right time.Operating Monday to Friday, 8:30am to 5:00pm, you will be responsible for screening, assessing, and supporting individuals into community-based and out-of-hospital care programs.
A key focus of the role is improving access across the Local Health District by ensuring timely, efficient, and equitable healthcare delivery.Working closely with a multidisciplinary team, you will provide expert care coordination and case management to facilitate timely, culturally safe, and well-coordinated access to services.
By guiding patients through their care journey, you will help prevent unnecessary and unplanned Emergency Department presentations and promote safe, effective out-of-hospital care through the Planned Care for Better Health program.Your contribution will strengthen continuity of care, improve patient outcomes,
