Since its establishment in ****, Alzheimer's WA has served as a prominent advocate and source of support for individuals living with dementia, as well as their families and carers.
We champion person-centred care, advocacy, education and comprehensive support through our Commonwealth Home Support Programme (CHSP), Support at Home (SAH) and day and overnight respite services across Perth, Mandurah and Albany.

About the Role
We are seeking an experienced and compassionate
Care Coordinator
to join our team in
Mandurah
on a
6-month fixed-term contract
, providing coverage for a period of leave.
This is a varied care coordination role supporting clients across our Support at Home (SAH), Commonwealth Home Support Programme (CHSP) and other community-based services.
Working closely with clients, families, Support Workers, scheduling, clinical staff and external service providers, you will coordinate supports that are responsive to each client's individual needs, goals and circumstances.

Key Responsibilities
Coordinate services and supports across SAH, CHSP and other community programs.
Support client onboarding, assessments, care planning and regular reviews.
Develop and maintain individualised support plans in partnership with clients and their families.
Coordinate with Scheduling, Support Workers, Registered Nurses and external providers to ensure continuity of care.
Monitor client wellbeing and respond appropriately to changes in needs or circumstances.
Maintain accurate client records, documentation and service information.
Provide clients and families with clear information regarding services, budgets, costs and available supports.
Ensure services are delivered in accordance with program requirements, organisational policies and the Aged Care Quality Standards.
Liaise with My Aged Care, referrers and other external stakeholders as required.
Contribute to quality improvement and positive client outcomes.
About You
To be successful in this role, you will have:
Previous experience in care coordination, case management or a similar role within aged care, community services or disability.
Understanding and experience working with
Support at Home and/or CHSP
.
Experience coordinating services and working with clients with varying support needs.
Strong organisational and time management skills, with the ability to manage competing priorities.
Excellent communication and relationship-building skills.
Demonstrated empathy and understanding when working with people living with dementia.
Strong attention to detail and the ability to maintain accurate client records.
Ability to work independently while also contributing effectively to a collaborative team.
A commitment to person-centred care and achieving positive outcomes for clients.
Certificate IV or higher qualification in Aged Care, Community Services, Individual Support or a related field.
Experience with My Aged Care and aged care funding processes.
Knowledge of aged care quality and compliance requirements.
