About the Opportunity
An exciting opportunity exists for a Clinical Nurse Specialist Grade 2 – Patient Flow Access Nurse to join the team at Wagga Wagga Base Hospital.
In this senior clinical role, you will provide expert nursing leadership and coordination to support safe, timely and effective patient flow across the hospital. Working collaboratively with patients, families, nursing and medical teams and key hospital services, you will identify and address barriers to access, admission, transfer and discharge.
You will use your advanced clinical knowledge, communication and problem-solving skills to support the right patient receiving the right care, in the right place, at the right time.

In this role, you will:

- Provide advanced clinical nursing leadership to support safe, timely and effective patient flow across Wagga Wagga Base Hospital
- Undertake comprehensive clinical assessment and support appropriate patient placement, admission, transfer and discharge
- Identify and proactively address barriers to patient access, flow and timely discharge
- Coordinate complex patient journeys in collaboration with medical, nursing, allied health and operational teams
- Provide expert consultation, advice and support to clinical teams regarding patient flow, access and escalation
- Act as a clinical resource, mentor and support for nursing and multidisciplinary staff
- Monitor patient flow, identify emerging risks and implement strategies to optimise hospital capacity and patient outcomes
- Promote effective communication and collaboration across inpatient, emergency, community and support services




- Lead and contribute to evidence-based practice, quality improvement and service development initiatives
- Advocate for patients and families, ensuring care is coordinated, person-centred and delivered in the right setting at the right time

About You
Our ideal candidate will demonstrate:

- Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered Nurse
- Demonstrated clinical experience and advanced nursing knowledge relevant to patient flow, access and care coordination with relevant please write up at selection criteria questions post graduate qualifications
- Demonstrated ability to coordinate complex patient care and effectively manage competing priorities in a fast-paced environment
- Robust clinical leadership, critical thinking, problem-solving and decision-making skills
- Excellent communication, negotiation and collaboration skills, with the ability to work effectively across multidisciplinary teams
- Demonstrated ability to identify barriers to patient flow and develop innovative solutions to improve access and patient outcomes
- Commitment to evidence-based practice, quality improvement, service development and ongoing professional development
- Demonstrated ability to provide clinical education, mentorship and support to nursing and multidisciplinary staff
