Austin Health is seeking a Nurse Practitioner for the Older Persons Complex Care (OPCC) rapid-response service. The role blends home visits with telehealth to assess, provide specialist geriatric care, and offer short-term clinical monitoring for older adults with complex health needs.

You will work in a team-oriented, multidisciplinary team to support timely discharge from hospital, coordinate with GPs and other services, and promote independence in the community.
