Vacro is seeking a dedicated Case Manager for the ReConnect program, delivering outreach support to participants transitioning from men's prisons across Victoria.
The role emphasizes relational, strengths-based, desistance-informed practice and seamless pre- to post-release care.
You will engage participants in custody and on release, coordinating plans with services, maintaining records, and addressing risks to ensure successful community reintegration.
