Job Description
Epworth HealthCare's Epworth at Home team in Richmond, VIC, seeks a Hospital in the Home Coordinator to oversee patient journeys into HITH and CITH, delivering acute hospital-level care at home.
You will provide clinical leadership, coordinate transitions and work with multidisciplinary teams to optimise patient outcomes.
The role requires current AHPRA registration as an RN, HITH experience, and solid knowledge of private health funding processes.
A postgraduate qualification is desirable.
