COMM CARE

Williamstown

commcare.health is a home care organisation founded by doctors who are committed to supporting older people. After seeing many elderly patients become housebound and lose access to timely care, our mission became clear: to integrate healthcare with home care so older persons can remain independent at home for as long as possible.

Our values are Teamwork, Kindness and Creativity, and we uphold high standards in every interaction, focusing on one client at a time.

commcare.health is eeking a proactive and compassionate Care Partner to join our home care team. In this role, you will work closely with older people and their families to ensure they receive the right support to maintain their independence, well‑being and quality of life at home.

As a Care Partner, you will:

Build meaningful relationships with clients to understand what matters most to them.

Conduct holistic assessments that identify strengths, needs, risks and personal goals.

Develop, implement and regularly review personalised care plans focused on wellness and reablement.

Coordinate and schedule services in partnership with the client, ensuring continuity, quality and value for money.

Monitor client outcomes and adjust support as needs evolve.

Work collaboratively with GPs, allied health professionals, community supports and family members.

Educate and empower clients to manage their own health and safety where possible.

Advocate on behalf of clients to ensure timely access to the services and equipment they require.

This role is central to delivering the Support at Home vision: a fairer, more personalised and proactive home care experience that helps older Australians live healthier, more independent lives.
